We will need the following in order for our consideration of your child.
- Script/Referral from a MD or DO with the child’s FULL name, DOB, Diagnosis AND Diagnosis CODE(s), and must be signed by the MD or DO and dated within a year from service request
- (CDE) Comprehensive Diagnostic Evaluation that included the testing and testing scores that were conducted that determined the diagnosis.
- Psychoeducational Report
- IEP if they need services in the school setting
- Copy of the front and back of the insurance card
